The simple meaning is that they are the ones who have followed you, but you have not followed them back yet.
How to See Accounts You Don’t Follow Back On Instagram
This feature works for both Android & iPhone devices.
Step 1: Launch the app on your device.
Step 2: Head to your profile by tapping your profile icon in the bottom-right corner.
Step 3: Click on the Followers.
Step 4: Now, under the Categories section at the top, tap Accounts You Don’t Follow Back.
You can now view the list of your followers you don’t follow back.
How to Follow Them Back
Once you’ve reached the list, tap Follow icon next to their username, and they will be added to the list of your followings.
Or, you can open their profile by tapping on it, then tap the Follow Back button.
How to Remove Them
Some of your followers may be inactive, or they don’t engage much with your posts and stories. In such cases, you may want to remove them.
On the lists of Accounts You Don’t Follow, just tap the Remove icon next to the profile you wish to remove. Then you need to confirm your decision by tapping the Remove again.
Note: When you remove a follower, Instagram doesn’t notify them. However, they can see whether they are following you by checking their following lists. Or, if they visit your profile and do not see the following icon that always appears if we have followed someone, they will know that they have been removed.

On your computer browser, open Instagram.com and log in to your account if aren’t logged in.
Tap your profile icon in the upper right corner of the screen, then select Profile.
Tap Followers.
The profiles which have Follow icon next to their username are the accounts you don’t follow back.
Tap Follow if you want to follow them or Remove to make them unfollow you.
